The infamous geiger counter bomb. When the IMF found this, they brought it back to their base and Mother Hen put it under his desk.
They didn't open it and notice the C4 and VX vials hidden inside. At 9:15 it "exploded", taking out the entire command bunker, including the IMF general. This bit of Black Rain/DBD deceit was worth 50 points to the Cabal. The base could not be used for re-insertions for 30 minutes.
